Don't expect IPOs

I think the public markets will remain closed in 2018 for health-tech companies. However, I do expect 2019 and the years beyond that to start to look more fruitful for initial public offerings, as well-funded start-ups like Health Catalyst, Flatiron Health, Welltok, Grand Rounds Health and others start to scale beyond $100 million in revenue and some of them start to reach positive EBITDA levels.

Healthcare mergers and acquisitions will remain robust but will focus more around health-care service businesses — many of which may be tech-enabled — rather than pure health-care IT companies.

Start-ups need to do more

I'm bullish on so-called "full-stack" or vertically-integrated health-care companies continuing to thrive. And by that I mean, companies that provide all of the necessary services and support around conditions like diabetes or the palliative care experience.

These services are the ones that are getting real engagement from patients and they are able to go to risk-bearing entities with a pitch to take a population off their hands -- and provide better care more affordably. These solutions will also work better for the patient as they take care of their totality of their medical issue.

The truth about artificial intelligence in medicine

Few could have missed the hype around artificial intelligence and machine learning technologies in health care in 2017, specifically related to fields like radiology and pathology.

A vast array of companies are developing algorithms to improve clinical care, but these will take time to develop commercially. In my view, 2018 will be the year that these technologies are instead applied to the "guts" of healthcare, meaning such unsexy applications as improving workflow, allowing doctors to spend more time on clinical care and less time on paperwork or operations, improving the operational efficiency of hospitals, and so on.

Biology isn't just a tech problem

2017 was the year that Silicon Valley started to invest in computational biology. A lot of these venture dollars were poured into companies focused on improving the early discovery process, such as identifying targets and "hits" of those targets.

I'm betting that 2018 will be the year where these companies and investors realize that biology is not "programmable" or "codeable." The problem isn't that pharma and biotech are short of "hits" or drug candidates. What these companies really need is clinical-stage programs. If these start-ups really want to scale and actually get traction among the traditional industry, they will need to become real biotech and drug development companies. And this will be a different, much more capital-intensive bet than their original investors probably realized.

Big tech will make moves

Apple, Alphabet and Amazon will continue to push forward in health care in 2018. If these companies make a real big splash with some kind of acquisition, it could crack open the IPO markets sooner than any of us think.

The year of humble pie

I have seen far too many companies in recent months that have raised behemoth rounds at mammoth valuations return to the market with their tail between their legs -- finally open to valuation adjustments. These could still be potentially successful companies, but their investors were potentially mistaken that such companies could reach hyper-scale.

5 Tech Trends That Will Rule 2018

There are publications, journals, books, week-long conferences (like CES) and endless amounts of information written about all of the incredible advancements in technology and innovation. For non-technical business owners, it's enough to make your head spin.

Here's a list that breaks it all down. This is not an academic, exhaustive review, but rather a quick skim at a very simplified level to give you an overview of what's top of the tech list for 2018. And there are always opportunities to learn more.

Data

Everything from this point out will be of, about and for data. Collecting it, storing it, securing it, sharing it. There are ample opportunities at every turn. Data can benefit you in two main ways: 1. owning information about individuals is valuable and can be monetized, 2. having insight helps you make smarter decisions.

Marketing will be more personalized than ever before. Those companies that can connect with their consumer (no matter how niche) and make them feel "you get me" will capture more market share. This can also be applied to the city level. Cities who are collecting and using data intelligently will engage and delight their residents and visitors, and redefine the urban experience.

Information Week also points out that the importance of intelligence is increasing while the price of the data is decreasing. This is great news for entrepreneurs and small businesses who don't have enterprise-level resources to unlock the insights they need to grow.

Blockchain

There is a lot of confusion about Blockchain, mostly thanks the speculation around BitCoin (which depends on blockchain technology). In its truest essence, Blockchain is about trust. Information is held and secured by a distributed ledger that is uneditable. In other words, reality is made by many, not by a single entity.

Judd Bagley breaks it down, "Blockchain provides the ability to create value and authenticate information." This has major implications on financial systems, identity management, supply chain automation and of course the sharing economy.

Of course it is much more complicated than this and Blockgeeks gives a great step-by-step guide. Also Inc.com contributor Bill Carmody gives an excellent overview in this piece, 4 Revolutions Made Possible by the Blockchain Economy.

Whether you think it's the next best thing or a bunch of hype, it is worth it to investigate what the buzz is all about. And if you think it's a niche technology for cryptogeeks, think again. The government is getting involved as regulators look to accelerate adoption.

Artificial Intelligence

Noted AI expert Amir Husain often says the phrase, "software is eating the world, and AI is eating software." Which basically equates to the notion that Artificial Intelligence will transform everything and has gone mainstream.

At its core, AI is the ability of machines to "learn," leveraging data, the cloud and software to compute at speeds that are far beyond human capability." If you've used voice-command or facial recognition technology, you're in the AI world. It's basically enabling machines to do a better job of what humans do.

ation to privacy. There will certainly be major change in every industry. However, this is no cause for alarm but instead a call to prepare, educate and adjust. We are in the 4th Industrial Revolution and the technology is finally ready to enhance the human experience in meaningful ways.

A few years ago, the top trend was mobile. From payment systems to mobile-ready web design, everything was about communicating to people via a handheld device. Now mobile is an assumption and the underlying technology - the network - is grabbing headlines.

This is the year of 5G, the next generation of mobile broadband that promises speeds up to 100 times faster than the current 4G technology. This is important because it fuels the Internet of Things (IoT), Artificial Intelligence (AI) and big data work which are working to automate our daily experience.

This level of innovation requires massive amounts of bandwidth and it is critical to not only have availability, but also low latency, which means lightning-fast speed. In a world with autonomous vehicles that depend on instant connectivity, the last thing you want is a delay in response time.

Other Foundational Technologies

Underlying all of these technologies are important concepts like security, privacy, software and the cloud. These area are no longer an afterthought or reaction for large enterprise-level organizations. Instead they are important parts of the planning phase for any level of technology implementation.

The major theme for 2018 seems to be that everything is connected. Carolina Milanesi with Creative Strategies goes on to say, "It's less about the hardware, and more about what's inside."

Bio-based compound offers a greener carbon fiber alternative

From cars and bicycles to airplanes and space shuttles, manufacturers around the world are trying to make these vehicles lighter, which helps lower fuel use and lessen the environmental footprint.

One way that cars, bicycles, airplanes and other modes of transportation have become lighter over the last several decades is by using carbon fiber composites. Carbon fiber is five-times stronger than steel, twice as stiff, and substantially lighter, making it the ideal manufacturing material for many parts. But with the industry relying on petroleum products to make carbon fiber today, could we instead use renewable sources?

In the December 2017 issue of Science, Gregg Beckham, a group leader at the National Renewable Energy Laboratory (NREL), and an interdisciplinary team reported the results of experimental and computational investigations on the conversion of lignocellulosic biomass into a bio-based chemical called acrylonitrile, the key precursor to manufacturing carbon fiber.

Acrlyonitrile is a large commodity chemical, and it's made today through a complex petroleum-based process at the industrial scale. Propylene, which is derived from oil or natural gas, is mixed with ammonia, oxygen, and a complex catalyst. The reaction generates high amounts of heat and hydrogen cyanide, a toxic by-product. The catalyst used to make acrylonitrile today is also quite complex and expensive, and researchers still do not fully understand its mechanism.

"That's where our study comes in," Beckham said. "Acrylonitrile prices have witnessed large fluctuations in the past, which has in turn led to lower adoption rates for carbon fibers for making cars and planes lighter weight. If you can stabilize the acrylonitrile price by providing a new feedstock from which to make acrylonitrile, in this case renewably-sourced sugars from lignocellulosic biomass, we might be able to make carbon fiber cheaper and more widely adopted for everyday transportation applications."

To develop new ideas to make acrylonitrile manufacturing from renewable feedstocks, the Department of Energy (DOE) solicited a proposal several years ago that asked: Is it possible to make acrylonitrile from plant waste material? These materials include corn stover, wheat straw, rice straw, wood chips, etc. They're basically the inedible part of the plant that can be broken down into sugars, which can then be converted to a large array of bio-based products for everyday use, such as fuels like ethanol or other chemicals.

"If we could do this in an economically viable way, it could potentially decouple the acrylonitrile price from petroleum and offer a green carbon fiber alternative to using fossil fuels," Beckham said.

Beckham and the team moved forward to develop a different process. The NREL process takes sugars derived from waste plant materials and converts those to an intermediate called 3-hydroxypropionic acid (3-HP). The team then used a simple catalyst and new chemistry, dubbed nitrilation, to convert 3-HP to acrylonitrile at high yields. The catalyst used for the nitrilation chemistry is about three times less expensive than the catalyst used in the petroleum-based process and it's a simpler process. The chemistry is endothermic so it doesn't produce excess heat, and unlike the petroleum-based process, it doesn't produce the toxic byproduct hydrogen cyanide. Rather, the bio-based process only produces water and alcohol as its byproducts.

From a green chemistry perspective, the bio-based acrylonitrile production process has multiple advantages over the petroleum-based process that is being used today. "That's the crux of the study," Beckham said.

XSEDE's Role in the Chemistry

Beckham is no stranger to XSEDE, the eXtreme Science and Engineering Discovery Environment that's funded by the National Science Foundation. He's been using XSEDE resources, including Stampede1, Bridges, Comet and now Stampede2, for about nine years as a principal investigator. Stampede1 and Stampede2 (currently #12 on the Top500 list) are deployed and maintained by the Texas Advanced Computing Center.

Most of the biological and chemistry research conducted for this project was experimental, but the mechanism of the nitrilation chemistry was only at first hypothesized by the team. A postdoctoral researcher in the team, Vassili Vorotnikov of NREL, was recruited to run periodic density functional theory calculations on Stampede1 as well as the machines at NREL to elucidate the mechanism of this new chemistry.

Over about two months and several millions of CPU-hours used on Stampede1, the researchers were able to shed light on the chemistry of this new catalytic process. "The experiments and computations lined up nicely," Vorotnikov said.

Because they had an allocation on Stampede1, they were able to rapidly turn around a complete mechanistic picture of how this chemistry works. "This will help us and others to develop this chemistry further and design catalysts and processes more rationally," Vorotnikov said. "XSEDE and the predictions of Stampede1 are pointing the way forward on how to improve nitrilation chemistry, how we can apply it to other molecules, and how we can make other renewable products for industry."

"After the initial experimental discovery, we wanted to get this work out quickly," Beckham continued. "Stampede1 afforded a great deal of bandwidth for doing these expensive, computationally intensive density functional theory calculations. It was fast and readily available and just a great machine to do these kind of calculations on, allowing us to turn around the mechanistic work in only a matter of months."

Next Steps

There's a large community of chemists, biologists and chemical engineers who are developing ways to make everyday chemicals and materials from plant waste materials instead of petroleum. Researchers have tried to do this before with acrylonitrile. But no one has been as successful in the context of developing high yielding processes with possible commercial potential for this particular product. With their new discovery, the team hopes this work makes the transition into industry sooner rather than later.

The immediate next step is scaling the process up to produce 50 kilograms of acrylonitrile. The researchers are working with several companies including a catalyst company to produce the necessary catalyst for pilot-scale operation; an agriculture company to help scale up the biology to produce 3-HP from sugars; a research institute to scale the separations and catalytic process; a carbon fiber company to produce carbon fibers from the bio-based acrylonitrile; and a car manufacturer to test the mechanical properties of the resulting composites.

"We'll be doing more fundamental research as well," Beckham said. "Beyond scaling acrylonitrile production, we are also excited about is using this powerful, robust chemistry to make other everyday materials that people can use from bio-based resources. There are lots of applications for nitriles out there -- applications we've not yet discovered."
